HGV MEDICALS AND PSV MEDICALS
The requirements for HGV and PSV are currently identical. A medical is needed when first applying for an HGV licence. A further medical examination is needed at age 45 and then at 5 yearly intervals until age 65, when annual medical examinations are needed. The interval between having your medical examination and applying for licence renewal must not exceed three months. If there are any changes to your health during the time between examination and applying for your licence then you must inform the DVLA. This could potentially result in the need for a further examination.
The medical examination requires form D4. This can be downloaded from the DVLA using the link button below:

TAXI MEDICALS
The medical requirements for a taxi driver are identical to that of an HGV licence holder. A medical examination will be needed when first applying for a licence and at renewal. You will need to contact your local authority prior to booking a taxi medical and ensure you bring the relevant Licensing Authority Medical Assessment Form with you or the medical will not be able to be completed. You will also need to check with your local authority about their requirements reading this medical as some local authorities require taxi medicals to be performed by your own GP. Others may allow any doctor to perform the examination but will require that they have a copy of your medical records. A print out of your medical records can be obtained from your GP surgery and a charge is likely to apply.
